SUMMARY: The Health and Wellness Services Assistant will be responsible for providing support to Kavod Senior Life’s Health & Wellness department. The primary objectives of the role are to arrange appointments, coordinate with providers, manage reminders and maintain timely and accurate records.

ESSENTIAL DUTIES AND RESPONSIBILITIES Include the following (other duties may be assigned):

  • Maintain the master calendar of provider visits and patient Arrange appointments, provide residents and other patients of the H&W Center with routine reminders of scheduled appointments, complete necessary paperwork, and provide escorts as needed.
  • Stays apprised of service provider offerings and maintains a high level of customer service with residents, providers, staff, families and community members to ensure that Kavod’s standards for quality service are achieved.
  • Supports the Health & Wellness team with data entry of HWDs, posting appointments in PCC, data entry of sign in sheets for classes, application completion and submission, etc.
  • Provides H&W program support by; creating and distributing fliers, reminding residents, supporting set up and take down, ordering supplies.
  • Works with the Health & Wellness team and other staff to assist with special projects and/or requests. Attends regular staff meetings as scheduled by the director and/or Kavod’s leadership Completes necessary required training in a timely fashion.
  • Supports care coordinators by; helping residents gather appropriate support documentation and for SSDI/AND/HCBS/PTC applications, in addition, to completing applications as needed.
  • Assists residents in appealing denied applications with the support of the care coordinator.
  • Maintains confidentiality in accordance with HIPAA

 

QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. EDUCATION and/or EXPERIENCE: Must be 18 years of age or older with a high school diploma or general education degree (GED) and he/she must read, write and speak English. Demonstrated success working with detailed documentation and processes is essential. Working knowledge of Microsoft Office Suite is required. Experience working with the elderly is preferred. Russian language skills are also helpful but not required.

BUSSER SUMMARY:  Assist with preparing tables, assist with special events (such as birthdays or holidays), handle food and beverages in accordance with sanitary procedures and standards, and ensure compliance with all federal, state and local regulatory procedures regarding food service. Training is provided!

DISHWASHER SUMMARY:  Maintain proper kitchen and dining room set up, wash and sanitize all dishes, utensils and pans, empty trash containers regularly, maintain a clean and safe kitchen, assist with special events (such as birthdays or holidays), handle food and beverages in accordance with sanitary procedures and standards, ensure compliance with all federal, state and local regulatory procedures regarding food service. Training is provided!

SCHEDULE:  Sunday, 12:00pm – 4:00pm, Thursday & Friday, 3:30pm -7:30pm

SUMMARY:  Kavod seeks a Care Partner to join our assisted living team. QMAP certification is preferred. Reliability is a must. We are currently seeking a compassionate and attentive Part-Time Care Partner to join our team. This position is key in supporting our residents with the care they need to accomplish their activities of daily living. Assistance with dressing, personal grooming, bathing and/or assistance during meal times are essential functions of the role. English language skills required.

On-Call mornings/days/evenings

On-Call QMAP overnight shifts will receive $1.00/hr for shift differential.

We require employees to be vaccinated against influenza with certain limited exceptions.
